Clifford Beers is a mental health clinic serving children and families. Our mission is to provide integrated services addressing mental, physical, and social determinants in order to improve health, resiliency and quality of life for children, families and communities.
PURPOSECare Coordination is an integral part of the philosophy of care at Clifford Beers Community Care Center. The Care Coordinator will deliver a blend of bilingual, family-centered, innovative billable and non-billable services to families based on their acuity and needs. Services are delivered in person, in office and in the community to facilitate the families' engagement. As a member of an interdisciplinary team, the Care Coordinator will assist the family in developing solutions, identifying community resources and supports based on the family’s needs and help promote permanent connections that support the family within the community.
KEY RESPONSIBILITIES Interpersonal communication- Establishing rapport with family members, schools and providers
- Engaging, educating, empowering families in a manner appropriate in fostering relationships that lead to the family’s success
- Tailor interactions to fit the tone, volume, and appropriate language for the client’s age and environment
- Collaborate and participate in an interdisciplinary team related to client care; in conjunction with the Clinic’s larger team
- Regular participation in staff meetings, team meetings, case conferences, and grand rounds as a vital team member of the Clinic
- Developing a shared understanding of respective roles and responsibilities of team members to ensure efficient collaboration
- Facilitating and monitoring implementation of each Plan of Care developed by the family’s team
- Engage families in the community and regularly conduct home visits
- Linking multiple services, healthcare providers, and community resources to meet families’ needs
- Ensuring flow and exchange of information among family members, the team, and linked providers/supports
- Maintain an active caseload of families to meet yearly programmatic needs
- Accurately document work in the organization’s electronic health record and any other platform as required by program
- Meet billable expectations as directed by the supervisor.
- Processing and determining appropriate eligibility
- Flex work hours to offer 3 late nights (until clinic closure at 8p) for families who require services after 5p
- Ensure that documentation is provided to the family in their preferred language (Spanish)
- Perform related and appropriate duties as assigned
This role may require travel throughout the State of Connecticut.
