Physical Therapist

We are seeking a licensed Physical Therapist to provide personalized, in-home and senior living care to older adults, utilizing our Geriatric House Calls™ model. Responsibilities include conducting evaluations, developing and delivering evidence-based treatment plans, supporting patient mobility and independence, documenting in EMR, and collaborating with a multidisciplinary team. Telehealth and virtual care options are incorporated to enhance flexibility and patient outcomes.

The role offers autonomy to manage treatment schedules, foster strong patient relationships, and promote aging-in-place education. Qualifications include a valid license, degree from an accredited program, computer proficiency, and CPR certification. Physical demands involve lifting, repositioning, and assisting patients, as well as driving to patient locations.
