Oak Street Health seeks a Behavioral Health Specialist to provide integrated mental health services for adults on Medicare in underserved communities. In this role, you will conduct biopsychosocial assessments, develop person-centered treatment plans, and deliver evidence-based individual and group therapy. You'll collaborate closely with primary care providers, care managers, and social workers to support whole-person, preventative care. Bilingual Spanish skills are strongly preferred.

- Provide comprehensive biopsychosocial assessments for adult and older adult patients
- Deliver individual and group therapy using evidence-based modalities
- Develop and update person-centered treatment and care plans
- Collaborate with primary care providers and care teams on integrated care
- Conduct risk assessments and provide crisis intervention as needed
- Maintain accurate, timely clinical documentation in the EHR
- Coordinate referrals to community and social support resources
- Monitor patient progress and adjust treatment as clinically indicated
- Participate in case conferences, huddles, and interdisciplinary team meetings
- Support a patient-centered, inclusive culture that advances health equity
