About This Role

- Collect and analyze data relevant to patient health.
- Identify expected outcomes for individualized care plans.
- Develop and modify care plans to achieve expected outcomes.
- Implement identified care plans effectively.
- Coordinate care delivery and transitions.
- Promote health and a safe environment through education.
- Evaluate progress toward health outcomes.
- Education:
- Graduation from an accredited school of nursing; BSN preferred.
- Experience:
- Registered Nurse foundational role; encouraged to advance through the SPIRE clinical advancement program.
- Licenses &
Certifications:- Licensed and currently registered nurse in the State of Connecticut.
- Current BCLS certification or must obtain within one month of hire.
- Clinical specialty certification highly encouraged.
