Registered Nurse RN, Hamden Internal Medicine

Overview

Pro Health Physicians (CT), part of the Optum family, is seeking a Registered Nurse (RN) for Internal Medicine in Hamden, CT. This is a full‑time, 40‑hour per week position located at 2560 Dixwell Ave, Hamden, CT 06514.

• Department: Internal Medicine
• Schedule: Monday 7:30 AM – 6:30 PM;
  Tuesday–Friday 7:30 AM – 5:30 PM (8 hours × 5 days)

• Provide and document patient care such as giving injections and performing preventive screenings.
• Triage patients according to approved protocols and scheduling guidelines.
• Prepare patients for providers by collecting and recording clinical information.
• Educate patients regarding their illness or injury and provide instruction for self‑management.
• Carry out standing orders, authorizing prescription refills and ordering tests as per policy.
• Document all patient interactions and clinical activities accurately in the electronic health record (EHR).
• Conduct outreach to patients for appointments, communicate test results, and follow up to ensure plan of care adherence.
• Answer and document patient phone calls; take complete messages and route to providers via the EHR.
• Monitor task buckets, follow up on overdue orders/worklists, and monitor status of critical referrals.
• Complete annual CLIA competencies with Regional Nurse Managers.
• Support the Clinical Lead and serve as a resource to other clinical staff.
• Precept new clinical team members.
• Maintain open lines of communication with the care team and Regional Management.
• Maintain knowledge of coding, OSHA, CLIA regulations, and company policies.
• Adhere to patient safety initiatives and infection control regulations.
• Perform other duties as assigned.

• Active, unrestricted RN licensure in Connecticut.
• At least 1 year of nursing experience.
• Intermediate computer proficiency in Microsoft Office.

• BSN degree.
• Experience with an electronic health record (e.g., Epic).
• Phone triage experience.
• Experience working in a hospital or medical office.
• Knowledge of medical terminology.
