Job Overview

Signify Health, part of CVS Health®, is seeking a full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. You will conduct 45‑minute visits in members’ homes, review medication and medical history, perform a physical evaluation, and, when ordered by the health plan, run diagnostic tests such as diabetic eye exams and spirometry.

• Bring your heart into every visit, joining a national network of purpose‑driven clinicians dedicated to improving health outcomes.
• Travel locally and within licensed states as needed to visit patients at home.
• Conduct virtual visits when required.
• Provide all Signify Health services indicated, including Diagnostic & Preventive Services products (spirometry, DEE, bone density screening, etc.).

• Active, unrestricted license(s) in coverage area(s) for NPs or PAs.
• Board certification (required).
• Multi‑state licenses (preferred).

Pay range: $95,738.00 – $. The actual base salary or hourly rate will depend on experience, education, geography and other factors.
