Total Care, a part of LHC Group and a member of the Optum family, is seeking a Registered Nurse to provide and direct in‑home nursing care in Hamden, CT. You will collaborate with physicians, families and the care team to plan and coordinate the total plan of care and ensure continuity of services.
Requirements include current RN licensure in CT, a valid driver’s license and access to a reliable vehicle or transportation; CPR certification preferred; 1+ year RN experience preferred.
