Clifford Beers Community Care Center (CBCCC) is a mental health clinic serving children and families. Our mission is to provide integrated services addressing mental, physical, and social determinants in order to improve health, resiliency and quality of life for children, families and communities.

PURPOSE

Fatherhood Engagement Services provides intensive outreach; case management services and 24/7 Dads group programming to fathers involved with an open case with the Department of Children and Families. The purpose of this program is to enhance the level of involvement of fathers in their DCF case planning, provision of services and positive parenting. Fatherhood Engagement Services is a Department of Children, Families (DCF) funded program.

The Fatherhood Engagement Specialist position is a direct care position working with individuals and families referred by DCF to provide wrap-around and trauma informed service coordination. The Fatherhood Engagement Specialist will provide one-on- case management, not less than once per week, as needed to address needs emerging during the course of assessment and intervention. Another key role of the position is to facilitate the Parent Education Program, 24/7 Dad, which is a twelve week curriculum of 2 hour sessions that focuses on teaching parenting and relationship building strategies.

The role includes all facets of case management from initial assessment, identification of participant needs, case planning, prevention and reintegration interventions, and services, as well as facilitating links with the participant's natural support network and external providers.

KEY RESPONSIBILITIES
  • Ensuring completion of prescribed forms following standardized procedures and timelines for program referrals.
  • Attend training and utilize the Wraparound approach to work with assigned families; in-home and in the community.
  • Coordinate case conferences with the referring DCF Social Worker.
  • Obtain Wraparound Certification in training modules in order to uphold fidelity to the Wraparound model.
  • Ensuring and maintaining development of a balanced Family Team Meeting.
  • Providing direct care to adults, children and families as needed.
  • Participating in community and state training activities to promote individual professional growth and development around Wraparound Model and other Project Longevity initiatives.
  • Utilize selected technology platforms for conducting needs assessment, care planning development, referrals and documentation. Maintain policies and standards for data management, privacy and confidentiality, HIPAA, security, and reporting as required to maintain compliance.
  • Conduct outreach and engagement activities for fathers and their family including in the members home and settings in the community.
  • Develop, coordinate and manage family-centered Care Plan and Care Team Meetings with members/family using the Intensive Care Coordination model for Project Longevity.
  • Maintain a trauma-informed, anti-racist, equity approach to improve the health and well-being of children, families, neighborhoods and community.
  • Utilize Electronic Health Records to complete all necessary documentation written in a timely manner consistent with all standards and regulations.
