Background:

Flemingdon Health Centre (FHC) is an engaged and involved member of some of Toronto’s most dynamic neighborhoods;
Flemingdon Park, Fairview and Thorncliffe Park. FHC is a registered charity and an incorporated not-for-profit Community Health Centre (CHC), with a vision of Strong Healthy Communities. Across our three sites, we provide a range of health-related services based on the social determinants of health with extensive community engagement.

Position Summary:

As a member of Flemingdon Health Centre's integrated backbone team, this role will provide operational support to partner organizations across the North York and East Toronto Ontario Health Teams. The Digital Health Supervisor supports physicians, nurse practitioners, and clinical teams with EMR systems, digital health tools, workflow optimization, troubleshooting, training, data quality, and reporting across FHC, HATP, partner primary care practices, and community sites.

The role requires regular travel across North York and East Toronto to provide in-person support, onboarding, and training.

Responsibilities include:

EMR & Digital Health Support
  • Provide front-line support to physicians, nurse practitioners, and clinical staff regarding EMR-related issues, workflows, and digital tools
  • Assist providers with EMR-related queries, troubleshooting, and workflow optimization
  • Resolve data quality errors, manage lab workflow issues, and maintain accurate provider/client information within EMR systems
  • Support implementation and optimization of digital health tools and provincial systems including referrals, lab integrations, and reporting tools
  • Provide in-person support and technical assistance at clinic and community sites as required
  • Maintain consistent practices across sites and support standardization of workflows and processes
Training & Onboarding
  • Train new providers and clinic staff on EMR systems, workflows, templates, and required documentation practices
  • Develop educational materials, workflow guides, and tip sheets for providers and staff
  • Support onboarding activities related to EMR access, system navigation, and digital health tools
  • Promote adoption and optimization of digital health solutions and workflow efficiencies
Decision Support & Data Quality
  • Assist with data quality monitoring, audits, validation, and clean-up activities
  • Support projects involving data sharing, reporting, multi-database integration, and quality improvement initiatives
  • Conduct data audits and support privacy investigations related to data quality or potential breaches
  • Assist with maintaining data integrity to support organizational reporting and decision-making
  • Support tracking of service requests, user access, asset management, and audit activities
Collaboration & Organizational Support
  • Collaborate with Decision Support team, providers, leadership teams, external partners, and other government organizations on digital health and quality improvement initiatives
  • Provide backup support to ensure continuity of internal operations and services
  • Contribute to policy and procedure development related to EMR workflows, data management, and privacy practices
  • Participate in organizational projects, meetings, committees, and training activities as required
  • Support continuous improvement initiatives and identify opportunities to enhance workflows and provider experience
  • Ensure client privacy, confidentiality, and compliance with organizational and legislative requirements
