Join our team at Accela Post Acute Care at Hamilton, a 55‑bed post‑acute care center located on the campus of RWJ University Hospital—Hamilton.

• Provide compassionate and personalized care to residents, ensuring their comfort and well‑being.
• Collaborate with the healthcare team to develop and implement individualized care plans.
• Administer medications, monitor health status, and provide education to residents and families.
• Foster meaningful relationships with residents and their families to create a welcoming environment.

• Current LPN license in the state of New Jersey.
• Experience in acute care, long‑term care, or similar settings required.
• Active CPR certification preferred.
• Effective verbal and written English communication skills.
• Highest level of professionalism with the ability to maintain confidentiality.
• Ability to communicate at all levels of the organization and work well within a team.
• Customer service-oriented and able to work well under pressure.

Weekend Requirement:
This position requires rotating weekends on/off. All per‑diem staff must commit to working at least one full weekend per month (both Saturday and Sunday).
