Affinity Health Management is seeking a Hospice On Call RN to respond to patient census and family needs after hours, nights, weekends and holidays.
The RN will perform urgent clinical and psychosocial assessments, manage pain and symptoms, document thoroughly, and collaborate with the Hospice Medical Director and the care team to ensure continuity and positive patient outcomes.
Applicants must hold a current NJ RN license and be willing to travel to patient homes as required.
