Registered Nurse (RN) Hospice - Full Time, Dayshift Mon-Fri
OverviewGentiva seeks compassionate, RN case managers to provide personalized hospice and palliative care to patients and families in home, assisted living, and other settings. The role focuses on skilled nursing, assessment, care planning, and interdisciplinary collaboration to support dignity, comfort, and love during end‑of‑life care.
Key Responsibilities- Serve as the primary point of contact for patients and families, guiding them through the hospice journey.
- Perform initial and ongoing assessments to monitor patient condition and adjust care plans.
- Deliver skilled nursing interventions and symptom management according to individualized plans.
- Collaborate with interdisciplinary teams—including physicians, LPNs, CNAs, social workers, chaplains—to refine care plans.
- Provide emotional, physical, and spiritual support to patients and loved ones, including education on medication, pain management, and end‑of‑life processes.
- Teach caregivers safe and confident care techniques.
- Maintain accurate, timely clinical documentation and follow infection control and compliance standards.
- Support quality improvement, scheduling, and continuity of care initiatives.
- Participate in on‑call and patient support activities as required.
- Comprehensive patient assessments and reassessments, delivering education to patients, families, and caregivers.
- Coordinate interdisciplinary care and communication, including with physicians and hospice providers.
- Document clinical records, including assessments, physician orders, progress notes, and care plans.
- Recognize and support transitions in levels of care and home‑health referrals.
- Maintain effective relationships with the interdisciplinary group for integrated outcomes.
- Education: Registered Nurse (BSN preferred).
- Experience: Minimum one year as an RN in acute care, oncology, long‑term care, hospice, or home health; hospice or palliative experience preferred.
- Licenses & Certifications: Current state RN license; CPR certification; valid driver’s license and automobile insurance.
- Knowledge & Skills: Knowledge of terminally ill patients and hospice care, assessment skills, documentation, regulatory guidelines, community resources, and interdisciplinary collaboration.
- Personal Traits: Compassionate, flexible, effective communicator, capable of supporting patients and families in crisis.
Salary Range: $100,000 – $115,000 annually. Compensation may vary based on location, experience, licensure, and other factors.
