Job Summary:
The Community-Based Care Coordinator assists the Behavioral Health Manager in effectively advocating and coordinating services and resources for individuals experiencing behavioral health related challenges. This is achieved by providing support and coordination to individuals after calls for service, discharge from incarceration, transitions from Bitterroot Health and/or West House and re-entry into the community. The Care Coordinator will work closely with community partners and the Ravalli County Behavioral Coalition.
This position is grant funded and dependent on continued future funding.
Essential Functions:
- Advocate for clients
- Organize referrals. Experience and skill in establishing and maintaining effective working relationships with various governmental entities, law enforcement agencies, criminal justice system staff, citizens, elected officials, and businesses.
- Identify and coordinate client goals, strengths, and needs by providing outreach and support in attaining client goals.
- Assist in establishing care with medical and psychiatric providers.
- Assist in locating information and resources for short term monetary needs.
- Ability to prepare accurate and concise data reports.

Supervisory Responsibilities:
None
Travel Required:
Regularly within Ravalli County. Occasional in-state and/or out-of-state travel for meetings and trainings.
Preferred
Education and Experience:
High school diploma, and any combination of education and experience that qualifies the employee to deal with social work, human services, crisis de-escalation, and behavioral health clients.
Work Environment:
Commonly works in an office environment with significant community outreach and engagement situations.
Physical Requirements:
Prolonged periods of sitting at a desk and working on a computer. Must be able to lift up to 20 pounds.
