What you will do:

  • Be an integral part of the Community Mental Health Team, delivering secondary care to older people with organic and functional mental health challenges.
  • Engage in comprehensive assessments, and devise short, medium, and long-term care plans, providing essential support and education to service users with a diverse range of psychiatric conditions.
  • Employ your expertise to guide those who do not require secondary mental health intervention toward alternative services.
  • Develop and implement treatment plans for individuals facing both organic and functional mental health issues.
  • Provide compassionate face-to-face and telephone support, addressing the unique needs of each service user sensitively.
  • Collaborate with senior team members to mentor junior staff and maintain a harmonious team dynamic through supervision and shared caseload management.
  • Keep accurate and current records in alignment with the Trust’s policy, ensuring the highest quality of care and accountability.
