Physical Therapist

We are seeking a licensed Physical Therapist to provide personalized, in-home and senior living community care, focusing on improving mobility, independence, and quality of life for older adults.

• Conducting evaluations, creating individualized treatment plans, and delivering evidence-based therapy sessions
• Supporting patient wellness, functional independence, and aging in place
• Documenting care accurately within an EMR system
• Collaborating with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists
• Educating patients, caregivers, and staff on safety and functional strategies
• Integrating telehealth and virtual care to enhance access and flexibility

• Valid Physical Therapy license or eligibility in the practice state
• Degree from an accredited PT program
• Proficiency with computers and EMR systems
• Current CPR certification

Physical requirements: Ability to perform moderate physical effort, assist with patient transfers, reposition patients, and drive to patient locations, while adhering to PPE protocols when needed.
