Job Overview

The Transfer Center Coordinator RN provides comprehensive care coordination for patients, initiating and managing both incoming and outgoing transfers to ensure timely and appropriate level of care. This role involves securing physician and specialist acceptance, performing clinical assessments, triage, arranging transport resources, and overseeing related administrative tasks such as patient registration, bed placement, insurance authorizations, and coordination of patient transport across network facilities.

Responsibilities
  • Prioritize patient transfers based on acuity levels.
  • Coordinate patient placement among multiple network facilities for admissions and transfers.
  • Ensure safe patient transfers according to established operational processes and guidelines for all service lines.
  • Evaluate medical necessity and appropriateness of care, optimizing patient outcomes.
  • Facilitate patient throughput to a final destination facility with focus on quality and efficiency.
  • Alert staff at receiving entities of incoming/outgoing patients, schedule beds/services, perform clinical and financial screening, and coordinate transportation.
  • Liaise with departments, leaders, and presiding physicians on process‑improvement opportunities for patient flow and communication.
  • Manage individual patients across the healthcare continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes.
  • Maintain referral pattern assessments, identify and cultivate opportunities for growth in assigned communities.
  • Collect and communicate pertinent information to payers and others for utilization and regulatory requirements.
  • Provide outcome data to physicians, case managers, rehabilitation professionals, insurance adjustors, and industry representatives.
  • Act as a resource and promote collegiality among staff physicians and leaders.
  • Participate in interdisciplinary healthcare teams and advise on integrated managed care concepts.
  • Optionally supervise staff depending on assignment.
  • Maintain professional knowledge through workshops, publications, and professional societies.
  • Ensure compliance with regulatory requirements such as EMTALA, HIPAA, and quality initiatives.
  • Use Central Logic database, telephone systems, Bed Management/Registration systems, and other office‑oriented systems to support operations.
  • Meet agent productivity standards set by Transfer Center Leadership.
Qualifications
  • Active BSN RN license in state of practice, temporary RN license, or compact RN licensure.
  • Critical Care Experience (ALS/PALS/BLS) with at least three years in care of the population served and experience in clinical care operations, case management, or patient throughput.
  • Working knowledge of hospital operations, medical/nursing staff procedures, hospital and community resources.
  • Demonstrated critical thinking, conflict resolution, time‑management, and problem‑solving skills.
  • Excellent communication skills and ability to interact with internal and external customers.
  • Excellent organizational and interpersonal skills for building effective relationships.
Preferred Qualifications
  • Expert level proficiency in Microsoft Office.
  • Bachelor’s degree in nursing or healthcare related field.
  • Additional related education and/or experience.
  • Typing accuracy of at least 40 words per minute.
Scheduling Requirements
  • Provide 10 shifts of availability per calendar quarter, submitted in the scheduling portal by the 15th of the previous month.
  • Must work 6 of the 10 submitted shifts.
  • 12‑hour shifts.
