• Location: Trenton, NJ
• Department/Unit: Field Assessments
• Schedule: Full-time, Monday-Friday, 8:00 AM-5:00 PM EST
• Pay: $66,575.00 - $
• Residency: Must reside in New Jersey
• Travel: Up to 85% travel required within Cumberland/Atlantic and surrounding Counties

• Conduct face-to-face assessments with members to evaluate medical needs and overall wellness.
• Help members access vital services such as Adult Medical Daycare and Personal Care Assistance.
• Complete 10+ assessments per week using a holistic approach.
• Collaborate with your team and supervisor to overcome barriers and achieve goals.
• Use interviewing skills to engage members and uncover their health needs.
• Mentor new team members once you've mastered your role.
• Assist with developing a proactive plan of care to enhance outcomes.
• Assess the need for referrals to clinical resources and other interdisciplinary team members.

• Education: Associate Degree in Nursing required; BSN preferred.
• Experience: 1 or more years clinical experience (hospital, home health, or ambulatory care).
