Support Coordinator

• Assessing service needs and developing the New Jersey Individualized Service Plan (NJISP) for access to services.
• Liaison with clients, families, programs, and service providers.
• Establish achievable goals, outcomes, and service eligibility.
• Development of a Person-Centered Planning Tool (PCPT).
• Ongoing monthly monitoring of clients.
• Coordinate and facilitate team meetings off-site at either Residential and/or Day Programs to identify service needs.

• One year of proven experience working with adults with developmental disabilities.
• Excellent written and verbal communication.
• Reliable car and flexibility to attend meetings when needed during the day.
• Must pass criminal background and CARI check.
• A bachelor’s degree in psychology, sociology, social work, or related field.

Demonstrates expertise in developing Individualized Service Plans (NJISP) and Person-Centered Planning Tools (PCPT) for adults with developmental disabilities, while effectively liaising with clients, families, and service providers. Strong communication skills and the ability to coordinate team meetings are essential for assessing service needs and monitoring client progress.
