Case Manager (Full‑Time)

New Jersey Community Development Corporation is a multi‑faceted community development and social service agency providing opportunities to transform lives in Passaic County. Independence House is a transitional living program for homeless youth and youth aging‑out of foster care who are between the ages of 17‑21.

Responsibilities
  • Serve as a member of the Independence House Team committed to helping young people build on existing assets to make positive choices about their health and well‑being, academic achievement, and interaction with others.
  • Coordinate the implementation of Individual Service Plans (ISP) for each youth in accordance with agency ISP planning policies and procedures.
  • Establish and monitor the methods used to teach the youth new skills as per individual ISPs and work with program staff to provide services in accordance with established methods.
  • Coordinate and link each youth to off‑site support services as defined in ISP; plan transportation for the youths’ participation in off‑site services.
  • Complete all case management documentation and reports and maintain individual youth files.
  • Review medication logs daily and conduct weekly audits of medication logs to ensure compliance with medication administration policy.
  • Implement behavior modification plans established by the team to address discipline issues and consumers’ lack of adherence to program rules, as needed.
Qualifications
  • Masters Degree in social work or human services related field.
  • One to three years of experience working with young adults and adolescents, particularly at‑risk individuals.
  • Excellent organizational skills, demonstrated leadership ability, sound judgment, and ability to work effectively as a member of a team.
  • Ability to work collaboratively with community resources and excellent interpersonal, verbal and written communication skills.
  • Valid driver’s license.
  • Computer skills are essential, with experience using Microsoft Word and Excel preferred.
