• Partner with members and families to identify supports and services to achieve personal health and community living goals
• Collaborate with a team of clinical and social support professionals to address members  physical, behavioral health, and long-term service needs
• Conduct thorough face-to-face assessments of members to determine their current status and needs
• Authorize and confirm the provision of appropriate home and community-based services
• Maintain a valid driver s license and comply with vehicle operation guidelines as necessary

• Minimum of 3 years of experience in a clinical setting
• Active Registered Nurse license in Tennessee or a relevant unrestricted Master’s in Social Work license
• Exceptional customer service skills
• Must reside within Hamilton County
• Effective time management, organizational, and communication skills

• Previous experience working in a community health or social services environment
• Familiarity with Microsoft Office and basic PC skills
• Strong problem-solving abilities and critical thinking skills
• Experience in conducting assessments and developing care plans
• Knowledge of relevant health and wellness resources for members
