Tri Health Hospice is seeking a Social Worker to support patients and families in Cincinnati/Hamilton North Region. This field-based role involves coordinating psychosocial care, discharge planning, and education with the multidisciplinary team.
Monday–Friday, 40 hours with on-call coverage including some weekends and holidays. Qualifications include a Master’s in Social Work and licensure as a Social Worker.
