JOIN OUR TEAMCommunity Health Alliance is looking for an LPN to provide care in a Level 3.7 Withdrawal Management residential treatment facility. LPNs in our program work closely with RNs, Nurse Practitioners, Physicians, and Behavioral Health Technicians to deliver high-quality, trauma‑informed care in a structured residential environment. In this role, you will provide 24/7 medically monitored care to individuals experiencing acute withdrawal symptoms, ensuring safety, stabilization, and dignity throughout the detox process.

- 1st shift: 8:00am - 4:00pm
- 3rd shift: 12:00am - 8:00am
1st shift LPNs start at $30.00 per hour and 3rd shift LPNs receive a $4.00 per hour shift differential.

- Complete nursing tasks as directed by a registered nurse or medical provider.
- Provide medically monitored withdrawal management in accordance with ASAM 3.7 standards.
- Conduct nursing assessments, including vitals, withdrawal scales (e.g., CIWA/COWS), and ongoing symptom monitoring.
- Administer medications safely and accurately, including PRN withdrawal medications per protocol.
- Participate as part of an interdisciplinary team coordinating patient care.
- Monitor patient appointments and collect health information and vitals for medical provider.
- Educate patients on monitoring and managing symptoms, medication and health conditions.
- Collect blood, urine and other specimens as needed for laboratory testing.
- Administer injections, TB testing, vaccinations as ordered by medical provider.
- Complete health and symptomology screenings to aid in patient care.
- Coordinate with other medical facilities to obtain medical records for treatment of patients.
- Implement appropriate nursing interventions and actions according to established plan of care within scope of practice.
- Support clients through the withdrawal process with empathy, professionalism, and therapeutic communication.
- Document all care in the electronic health record (EHR) in compliance with state and federal regulations.
- Practice within the scope of practice as outlined by the Ohio Board of Nursing.
- Licensed in the State of Ohio as a Licensed Practical Nurse (LPN).
- Credentialed with provider type 95.
- Two years related experience in withdrawal management, behavioral health, residential treatment, or acute care preferred.
- Current BLS/CPR certification required.
- Valid driver's license and reliable transportation (travel between sites required).
- Ability to work flexible hours, including early morning, weekends or evenings if needed.
- Experience in substance use treatment, behavioral health or related nursing field preferred.
- Knowledge of medication-assisted treatment (MAT), harm reduction and trauma-informed care is preferred.
- Commitment to providing nonjudgmental, empathetic care to individuals with substance use and behavioral health disorders.
- Effective verbal and written communication skills.
- Proficient with Microsoft 365 applications, including Office Suite, Outlook and Teams.
- Adept at learning and navigating electronic health record systems.
