Hospice LPN Night

Join Addus Family Company as a Hospice LPN on our night shift in Fayetteville. Provide compassionate end-of-life care in patients' homes, including assessments, symptom and pain management, medication administration, and wound care. Collaborate closely with RNs, physicians, social workers, and chaplains to follow individualized care plans. Accurately document in the EMR, support families with education and emotional support, and uphold strict safety, infection control, and compliance standards.

Responsibilities

• Provide direct hospice nursing care to patients in their homes on the night shift
• Perform focused assessments, monitor symptoms, and report changes in condition
• Administer medications and treatments, including pain and symptom management
• Support and educate patients and families on end-of-life care and comfort measures
• Document assessments, interventions, and patient responses in the EMRCollaborate with RNs, physicians, social workers, and chaplains on care plans
• Follow infection control, safety, and regulatory compliance standards
• Participate in on-call rotation and respond to urgent patient needs at night
