GENERAL DESCRIPTION:

The WISH Housing & Community Resilience Coordinator supports youth ages 14–24 experiencing homelessness, housing instability, or significant stress and trauma by providing individualized housing navigation, care coordination, and community-based prevention services. This position combines direct housing stabilization services with community engagement responsibilities. Using a trauma-informed and strength-based youth development approach, the Coordinator assists youth in obtaining and maintaining safe, stable housing while strengthening protective factors and connecting them with education, employment, behavioral health, and other supports.

In addition to direct service responsibilities, the Coordinator builds collaborative partnerships with community organizations, landlords, and youth-serving agencies.

Additionally, this role will support community engagement, outreach & youth development initiatives through the Resilience in Communities After Stress and Trauma (ReCAST) grant. The coordinator will work closely with youth and partners in Hamilton, Middletown, and Fairfield to build community awareness of resources, complete listening sessions & photovoice, assist with event planning & management, and complete required reports.

This position will be focused 50% on youth & young adult homelessness and 50% on community resilience activities.

Essential Functions

Program Coordination & Compliance

  • Maintain accurate case records, documentation, and program data in accordance with grant requirements, including HMIS certification and data entry.
  • Prepare required reports and communicate program outcomes, trends, service gaps, and barriers to supervisors, funders, and community partners.
  • Participate in multidisciplinary team meetings and community collaborations to coordinate services and strengthen system partnerships.
  • Maintain confidentiality and comply with all agency, federal, state, and local regulations.
  • Complete required professional development and maintain proficiency in evidence-based and trauma-informed practices & HUD housing standards.
  • Perform other duties as assigned to support program and organizational goals.

Housing Navigation & Youth Support

  • Identify, engage, and assess youth ages 14–24 who are homeless, fleeing unsafe living situations, or at imminent risk of homelessness.
  • Conduct housing assessments and determine program eligibility.
  • Develop individualized housing stability plans with immediate and long-term goals that are updated monthly.
  • Assist youth in locating, applying for, and securing safe, affordable housing.
  • Recruit, develop, and maintain relationships with landlords, property managers, and housing providers.
  • Coordinate housing-related financial assistance including security deposits, rental assistance, utility assistance, application fees, and moving expenses in accordance with grant requirements.
  • Assist youth in completing housing applications, gathering documentation, understanding lease agreements, and developing budgeting skills.
  • Provide ongoing housing stabilization services including home visits, safety planning, crisis intervention, and problem-solving to promote successful housing retention.
  • Connect youth with education, employment, healthcare, behavioral health, substance use treatment, childcare, transportation, public benefits, and other community resources.
  • Transport youth within the company vehicle as needed.

Community Resilience & ReCAST Coordination

  • Assist in planning and implementing community resilience initiatives.
  • Build collaborative relationships with schools, behavioral health providers, community organizations, faith-based organizations, and other stakeholders.
  • Identify gaps in community services and assist in developing strategies that strengthen protective factors and improve community wellness.
