• Continued assessment of residents to promptly address any issues regarding the resident's health.
• Work with our multidisciplinary team to ensure the residents plan of care is followed and updated according to their individual needs.
• Communicate with third party providers and family to address any issues arising which would be applicable to their health and wellbeing.

• Compassion and a passion for helping others is a must.
• Multitasking.
• Excellent communication and interpersonal skills are a must.
• Excellent concept of HIPAA and confidentiality standards.
• Experience with PCC or similar EMAR system preferred.
• Licensed by the Louisiana Board of Practical Examiners in good standing.
• CPR/BLS.

Pay Rate – Starting base pay $24.00 to $27.00 per hour depending on experience.

This is a full time position. Pay Range: $24 to $27.
