Overview
Sub-Acute, Long-Term Care is a sector of healthcare exploding with potential and work-life balance flexibility (no on call or weekends).
We are seeking a Full Time (40 hours per week) provider for our Oceanside Center located in Hamopton, NH. This position can be filled by a nurse practitioner or physician assistant. New grads are welcome to apply.
The base salary range for this position is $112,320 - $128,960 / year, in addition to our no-cap incentive compensation bonus program.
ResponsibilitiesThe Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager. The Advanced Practice Provider collaborates on‑site with the Center Leadership Team for center specific goals and priorities, and with the Medical Director and/or Attending Physician for patient specific medical care.
The Advanced Practice Provider provides primary and urgent care services to nursing center residents in collaboration with the other medical providers in the center, working collaboratively with Center Leadership to achieve Align Med Partners mission of improving the lives we touch through high quality health care and everyday compassion.
QualificationsThe Advanced Practice Provider must be a graduate from an accredited School of Nursing program, licensed as a registered nurse, and certified as a Nurse Practitioner by being a nursing graduate from an accredited Nurse Practitioner Program with a Master’s Degree in Nursing, with a concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP). Other concentrations/specialty tracks may be considered based on experience and business need.
A Physician Assistant must be a graduate from a program accredited by the ARC‑PA, have taken the PA National Certifying Exam (PANCE) to use the PAC designation, and hold PA state licensure. NCCPA certification is required. A minimum of two years’ experience working with adult or geriatric populations is preferred. In states where practice allows, an APRN with an unrestricted professional license and DEA for each state in which they practice is required;
similarly a PA with an unrestricted professional license and DEA for each state in which they practice is required.
