Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)

We are seeking a dedicated Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) to join our multi-disciplinary center. The role involves providing quality nursing care in a supportive environment, with a focus on patient comfort and recovery.

• Administer medications and treatments accurately according to physician orders.
• Monitor patient vital signs and report any significant changes to the healthcare team.
• Document patient care activities and observe progress meticulously.
• Collaborate with team members to develop and implement individualized care plans.
• Ensure compliance with all safety, privacy, and infection control protocols.
• Maintain a professional and compassionate approach with patients and families.

• Valid LVN/LPN license in the State.
• Proven experience in LTC/SNF settings preferred.
• Ability to implement nursing care plans and evaluate patient needs effectively.
• Demonstrate excellent communication and interpersonal skills.
• Must have completed the COVID-19 vaccination series or hold valid exemption documentation.
• Ability to work flexible hours including weekends as required.
