Licensed Practical Nurse

River Woods Exeter is seeking compassionate RN/LPN nurses to provide high-quality care to residents in our continuing care retirement community. In this role, you will deliver skilled nursing services across independent living, assisted living, and nursing care settings, including assessments, medication administration, care planning, and coordination with an interdisciplinary team. You'll support residents and families with education and advocacy while promoting dignity, safety, and comfort.

Responsibilities

• Provide direct nursing care to residents in independent living, assisted living, and nursing care levels
• Conduct comprehensive resident assessments and document findings accurately in the electronic health record
• Administer medications and treatments per provider orders and facility policies
• Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team
• Monitor resident conditions, respond to changes, and communicate promptly with providers and families
• Supervise and support CNAs and other support staff, ensuring high standards of resident care
• Educate residents and families on health conditions, medications, and wellness strategies
• Promote a safe, respectful environment that supports resident dignity, independence, and quality of life
• Participate in care conferences, quality improvement initiatives, and regulatory compliance activities
• Follow infection control, safety, and emergency procedures at all times
