LVN​/LPN – LTC​/SNF

Job Summary:

Provide high-quality nursing care to residents in a long-term care or skilled nursing facility. Ensure patient safety, comfort, and well-being through effective clinical practices and adherence to facility policies.

Responsibilities:

• Administer medications and monitor residents' health status.
• Document patient assessments, interventions, and outcomes accurately.
• Collaborate with healthcare team members to develop and update care plans.
• Follow infection control protocols, including COVID-19 safety measures.
• Maintain compliance with regulatory and facility requirements, including  wearing and vaccination documentation.
• Communicate effectively with residents, families, and staff to coordinate care.

Qualifications:

• Valid LVN or LPN license in good standing.
• Proven experience working in LTC/SNF settings preferred.
• Ability to adhere to safety and infection control standards.
• Strong interpersonal and communication skills.
• Must bring a valid state or federal  wear a photo badge at all times.
• Completes required COVID-19 vaccination or has approved exemption documentation.
