Inpatient Case Manager - MSW

Sentara Careplex Hospital in Hampton, VA is seeking an experienced Inpatient Case Manager MSW to focus on complex cases.
Job DetailsFull Time & Day Shift, 40 hours/week.
Schedule:
Monday – Friday 8:30am – 5pm, with weekend rotation approximately every 4 weeks, and holiday and on‑call commitments.
Responsible for the provision of medical social work services to patients receiving care in the hospital setting. Assesses the social, cultural, environmental, and financial situations, as well as the disposition needs of each referred patient. Collaborates with healthcare team members and community organizations to identify needs and resources in the hospital and community. In consultation with the healthcare team develops effective transition plans.
Collaborates with the RN Case Manager to ensure a smooth flow of patients through their hospital stay.
- Master's Level Degree in Social Work
- Certification in Case Management (CCM, CMAC or ACM‑SW) required within one year of eligibility
- BLS required within 90 days of hire
- 1 year of related acute care and/or healthcare experience required (educational internship experience in an acute care facility may be considered)
- Healthcare experience refers to care delivered in a hospital or an acute care facility

Sentara Careplex Hospital, located in Hampton, VA, is a 224‑bed technologically advanced acute care facility and Certified Primary Stroke Center. It offers specialized services in heart and vascular, urologic care, and advanced diagnostic and surgical capabilities, a hospitalist program, and one of the state's busiest emergency departments. The hospital also houses the Orthopedic Hospital at Sentara Care Plex Hospital, the area's first dedicated orthopedic hospital.
