Job TitleInpatient Case Manager, RN
Job SummaryResponsible and accountable for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum. Primary focus on the patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS. Directly support those in‑hospital patients identified in need of comprehensive case management. Promote effective utilization and monitoring of health services, collaborate and communicate with the healthcare team and patient/caregiver to manage care and transitions.
Develop and implement a comprehensive care plan based on assessment and evaluation to include the physical, mental, and psychosocial needs of the inpatient and caregiver. Functions within the hospital setting only.
BLS within 90 days of hire. BSN preferred. Case Management (ACM, CCM, CCCTM, CMAC, or CMGT‑BC) certification required within one year. One year acute care case management experience preferred.
Education- RN‑Associate's Degree
- RN‑Bachelor's Degree
- RN‑Diploma
- RN‑Master's Degree
- RN‑Doctorate Degree
- Registered Nurse (RN) License – Compact or Multi‑State License
- Registered Nurse (RN) License – Single State, Virginia
- Basic Life Support (BLS) Certification – American Heart Association
- 1 year of nursing experience
- 1 year acute care experience (preferred)
- 1 year case management experience (preferred)
