Sentara Careplex Hospital is hiring an Inpatient Case Manager, RN.
Shift: Days (8 hours).
Location:
Hampton, VA The ICM, RN position will support the Emergency Department and has responsibility for health plan members. It functions as part of the hospital’s integrated delivery system care coordination model. While operationally aligned to hospital leadership, the role shares accountability with health‑plan clinical leadership to support high‑risk member management, utilization reduction strategies, care transition performance, and enterprise population health objectives.
Responsible and accountable for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum. Primary focus is on patient transition to the appropriate level of care while ensuring clinical appropriateness and length of stay. Directly support hospital patients identified as needing comprehensive case management. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions.
Develops and/or implements a comprehensive care plan based on assessment and evaluation to include the physical, mental, and psychosocial needs of the inpatient/caregiver. Functions in a hospital setting only.
- RN‑Associate's Degree
- RN‑Bachelor's Degree
- RN‑Diploma
- RN‑Master's Degree
- RN‑Doctorate Degree
- Registered Nurse (RN) License – Compact/Multi‑State License
- Registered Nurse (RN) License – Single State – Virginia
- Basic Life Support (BLS) – American Heart
- Case Management certification (ACM, CCM, CCCTM, CMAC, or CMGT‑BC) required within one year
- AHA BLS – within 90 days of hire
- 1 year of nursing experience (required)
- 1 year acute‑care experience (preferred)
- 1 year case‑management experience (preferred)
