Job Overview
Physical Medicine and Rehabilitation (PM&R) or Physiatry specialist as an NP/PA working as a PM&R Clinician with Comprehensive Rehab Consultants (CRC). The role focuses on treating patients exclusively at a designated skilled nursing facility (SNF) or post‑acute rehab center.
Location and HoursHampton, VA. Full‑time 40‑hour W‑2 position. 8‑hour shift around 9AM–4PM with flexibility. 5‑day workweek with flexible days (Monday through Friday).
Compensation and Benefits- Base salary $112,000 – $132,000

- Laptop supplied
- Provide specialized PM&R/Physiatry services as a rehab consultant
- Represent CRC in the facility through strong clinical leadership and collaboration with the interdisciplinary care team
- Collaborate with PCP teams to manage conditions common in SNFs such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries
- Work alongside PT, OT, and SLP teams to improve patient function and quality of life
- Follow therapy patients to track progress in therapy
- Assess and collaborate on the management of barriers to therapy including joint or neuropathic pain, spasticity, neurogenic bowel, neurogenic bladder, and more
- Attend multidisciplinary meetings, bridging the gap between therapy and internal medicine
- Engage in culture‑building opportunities and ongoing education, including monthly didactic lectures, virtual regional team meetings, and annual regional dinners
- Passion for post‑acute healthcare and serving the geriatric population
- Experience in SNF or post‑acute settings (preferred)
- Confidence to function autonomously
- Desire for continuous improvement and ability to embrace change and growth
- Strong interpersonal skills and ability to understand the needs of facility partners and patients
- Empathy and compassion
- Commitment to investing in company culture
- Nurse Practitioner/APRN/Physician Assistant license
- Valid Virginia license
- Ability to work in the United States
- Experience in post‑acute space preferred
- Positive attitude and dedication to delivering quality patient care
- Availability for 40 hours/week
