Job SummaryThis position reports to the Program Director and is responsible for creating partnerships, linking individuals in need with services, and improving referral pipelines for VOAC’s Suicide Prevention services. The Community Outreach Case Manager will work a conventional week that includes evening hours and may require weekend work.
Responsibilities- Coordinate outreach efforts for the Veterans Program—including establishing an outreach plan for eligible participants, developing a schedule to visit the VA, shelters, and other gathering places, and maintaining communication with the VA and community providers.
- Screen potential program participants for eligibility using VA-approved forms, and follow up with agencies to verify veteran and income status.
- Provide SSG Fox SPGP services as outlined in the program guidelines:
- Outreach to identify those at risk of suicide
- Baseline mental‑health screening for participants over 18 years of age
- Education on suicide risk and prevention for families and communities
- Provision of clinical services for emergency treatment
- Case‑management services
- Peer‑support services
- VA benefits assistance for eligible individuals and their families
- Assistance with obtaining and coordinating other benefits from federal, state, local governments, or eligible entities
- Assistance with emergent needs such as health‑care services, daily living services, personal financial planning, transportation, temporary income support, fiduciary and representative payee services, legal services, and childcare
- Non‑traditional and innovative treatment practices as determined appropriate by VA
- Other services necessary to improve mental‑health status and reduce suicide risk of eligible individuals and their families
- Provide or arrange support and wrap‑around services, connecting individuals to all eligible VA and mainstream resources.
- Communicate effectively (under confidentiality rules) with service teams to address and correct issues with services or supports—particularly living skills, employment, education, family, and socialization.
- Participate in program and professional supervision, attend required staff and Project Management Team meetings and trainings, and contribute to team‑based collaborative planning.
- Complete other duties as needed and requested.
- Attend monthly community outreach meetings.
- Assist neighbors experiencing homelessness with accessing resources and making referrals; continue engagement until shelter or permanent housing is obtained.
- Maintain computerized client records, daily activity logs, mileage logs, and other required reports.
- May drive and inspect a van/shuttle, clean it, and maintain all safety conditions.
- Develop relationships with neighbors through frequent outreach visits to foster trust and program participation.
- Research community resources that benefit veterans.
- Provide advocacy for veterans facing barriers.
- Foster integration of the program with existing departmental and regional services—homeless services, veteran’s services, behavioral health, etc.
- Travel throughout the community and local sites.
