- Newport News Community Services Board is seeking a Case Management Assistant to provide administrative and care coordination support for our Adult Mental Health Case Management team. As a Case Management Assistant, you’ll play an important role in helping individuals access services by supporting the daily operations of the Case Management program.
Responsibilities include:
- Complete intake screenings and coordinate appointments for new referrals.
- Assist with care coordination and service linkage for individuals receiving case management services.
- Maintain a caseload of non-billable clients and ensure timely follow‑up.
- Enter accurate and timely documentation into the electronic health record (EHR).
- Support Case Managers with administrative tasks and service coordination.
- Monitor and maintain intake, screening, and caseload tracking systems.
- Organize tasks, calendars, and communications using Microsoft Outlook.
- Maintain an up-to-date directory of community resources and referral information.
- Communicate professionally with individuals receiving services, families, hospitals, and community partners.
- Perform other administrative and program support duties as assigned.
- Associate’s Degree in Human Services (such as Social Work, Psychology, Sociology, Counseling);
- Three (3) years of clerical experience and at least six (6) months experience working with individuals living with serious mental illness.
This position offers a consistent Monday through Friday schedule from 8:30 a.m. to 5:00 p.m. Your day will include reviewing referrals, monitoring intake and caseload trackers, updating records, coordinating services, and supporting the Case Management team. You’ll help ensure information is accurate, referrals move forward efficiently, and individuals are connected with the resources they need. Your organization and attention to detail help keep services running smoothly and make a meaningful difference for the people we serve.
