- Report to the Program Director
- Create partnerships and linkages and identify individuals needing services
- Coordinate Veterans Program outreach, including outreach planning and scheduled visits to VA facilities, shelters, and community locations
- Develop communications and updates with the VA and community providers
- Screen potential participants for eligibility using VA-approved forms and verify veteran and income status
- Provide or arrange suicide prevention outreach, mental health screening, education, clinical emergency treatment, case management, peer support, and benefits assistance
- Connect individuals and families with VA, federal, state, local, and mainstream resources
- Assist with emergent health care, daily living, financial planning, transportation, legal, childcare, and related needs
- Communicate confidentially with the service team to address service and support issues
- Participate in supervision, staff and Project Management Team meetings, trainings, and collaborative planning
- Attend monthly community outreach meetings
- Assist people experiencing homelessness with accessing resources, making referrals, and pursuing shelter or permanent housing
- Maintain computerized client records, daily activity logs, mileage logs, and other reports
- Operate and maintain a van/shuttle when needed, including inspections and safety conditions
- Conduct frequent outreach visits to build trust and program participation
- Research community resources and advocate for veterans facing barriers
- Integrate the program with homeless services, veteran services, behavioral health, and other programs
- Travel throughout the community and local sites
- Provide community stabilization support and immediate mental health services during crises
- Help reduce hospitalization utilization, law enforcement contact, and recidivism
- Bachelor's degree in Human Services or related field required
- 3 to 5 years of experience working in outreach
- Experience working with Veterans or people who have mental health, substance use, or IDD concerns
- Ability to explain an agency's purpose, programs, and operations
- Knowledge of available community resources
- Mission-driven, self-starting approach
- Excellent computer skills in Microsoft Office and office products
- Excellent written and verbal communication skills
- Proven ability for conflict resolution
- Ability to safely operate a motor vehicle
- Ability to travel to assigned locations and perform required physical activities
- Bilingual in any language is a plus
Demonstrates expertise in outreach coordination, community resource integration, and veteran services, with a strong focus on mental health support and crisis intervention. Proficient in managing client records and providing comprehensive assistance to individuals facing barriers to care.
