DescriptionSEMHA is seeking a compassionate and motivated Community Health Worker (CHW) to join the Wayne FQHC Program located in the city of Hamtramck
. The Community Health Worker is a part of an inter‑disciplinary clinical care team at a health center who helps increase access to health services for people through outreach, encouraging engagement, education, and peer support.
- Conduct patient outreach and provide education on preventive screenings and chronic disease management.
- Address HEDIS care gaps and engage high emergency department utilizers to connect with their PCP.
- Perform Social Determinants of Health (SDOH) screenings and link patients to appropriate community resources.
- Coordinate referrals, assist with scheduling, and help remove barriers to care.
- Maintain accurate documentation, outreach tracking, and monthly reporting for Health Plans.
- Participate in community outreach events and support site visit preparation (e.g., HRSA, AAAHC).
- Maintain HIPAA compliance and confidentiality at all times.
- Support patient panel management and other duties as assigned.
- High school diploma required.
- Bachelor’s degree in health sciences or related field preferred.
- MICHW certification preferred.
- Experience in community work, education, or healthcare strongly preferred.
- Strong communication, organizational, and interpersonal skills.
- Ability to multitask in a fast‑paced environment and advocate for patients.
- Proficiency in Microsoft Office preferred.
- Valid driver’s license and reliable transportation required.
