Salary Range: $25.34 – $26.88 per hour

The Case Manager II provides rehabilitation, care coordination, and supportive services to adults receiving outpatient behavioral health services while promoting wellness and recovery.

• Provide rehabilitation services, coordinate care with treatment teams, conduct community-based services and home visits as appropriate, maintain documentation standards, collaborate with community partners, and support recovery-oriented treatment approaches.

• Qualified candidates must meet one of the education and experience combinations identified by the position requirements and possess a valid California Driver License with an insurable driving record.

• Behavioral health experience, knowledge of community resources, and experience working with adults experiencing mental health or co-occurring disorders are preferred.
