Full-Time Position for Substance Use Counselor.
JOB SUMMARYThe Community Support Specialist/Integrated Treatment Specialist is responsible for Integrated Treatment for Co-Occurring Disorders (ITCD) programming and may include managing a reduced caseload for community support duties. This includes providing substance use and group counseling and group education; reviewing and updating ITCD tracking logs; and case consultation with various CHS staff. In addition, community support provides access to services in the community with serious and persistent mental illness(es) and individuals with other co-occurring disorders such as substance use disorders, developmental disabilities, and other co-occurring problems that affect life functioning.
MAJORJOB ELEMENTS
- Present client cases in Supervision, Physician Consultation, and ITCD Staffing.
- Documents services that clearly describes the need for the service, the intervention provided, the relationship to the treatment plan, the provider of the service, the date, actual time and setting of the service, and the individuals response to the service.
- Working collaboratively with the individual on treatment goals and service including the useof collaborative documentation as a tool to ensure that individuals are active in their treatment.
- Developing, implementing, updating, and revising as needed the treatment plan that identifies specific, measurable and individualized interventions. This includes participating in treatment plan staffing and reviews.
- Developing a discharge and aftercare/continuing recovery plan to include, if applicable, securing a successful transition to continued services.
- Provide information and education to the individuals, families, and significant others regarding the types, availability and places of services which may assist in the individual’s rehabilitation. This includes locating and helping clients utilize community resources.
- Maintain a professional working relationship with other agencies as well as collaboration with consumer services provided within CHS (i.e. HCH)
- Monitor and assist as needed with individual’s medical concerns.
- Participates in CSS crisis on call service by supporting individuals in crisis situations including locating and coordinating resources to resolve the crisis.
- Provide assistance to individuals in a responsible and flexible manner. Demonstrate ability to prioritize consumer needs.
- Maintain contact with individuals who are hospitalized for medical or psychiatric reasons and participate in and facilitate discharge planning for psychiatric hospitalization and for medical hospitalizations as appropriate.
- Provide training in activities of daily living skills (i.e. housekeeping, cooking, personal grooming.)
- Provides individualized assistance to consumers to create a personal support system that includes family members, legal guardians, and/or significant others.
- Work with supervisor to establish goals to meet organizational expectations and consumer needs and is accessible during regular office hours.
- Maintain cleanliness of the work environment.
- Is responsible for participating in chart reviews as assigned and make necessary corrections as needed.
- Successfully meets and maintains all certification, training, and accreditation requirements as deemed necessary by the organization. This includes the maintaining the QAP credential
Meets all organizational billing expectations as well as timely submission of SDL’s and progress notes.
Provides group substance abuse counseling to individuals admitted to the ITCD program and educational group services to the families and significant others of the ITCD participants.
Administers the Addiction Severity Index (ASI) to individuals identified via the CAGE in priority determined by the ITCD Team/CHS administration and prepares the resulting report.
- Graduate from an accredited college or university with a Bachelor's Degree in social work, counseling, psychology, or closely related field; OR
- At least four years of relevant, qualifying full-time equivalent experience in human service delivery and demonstrates skills in developing positive and productive client relationships and the ability to negotiate complex service systems to obtain needed services and resources for clients; AND
- At least 23 years of age; AND
- Meets the criteria, including training requirements and certification of a Certified Reciprocal Alcohol Drug Counselor as a Qualified Addiction Professional.
