Responsibilities
River Oaks Hospital (a UHS affiliated hospital) in metropolitan New Orleans, Louisiana, is a private psychiatric facility for adults, adolescents, and children. We are seeking a dynamic and talented CASE MANAGER / DISCHARGE PLANNER to support the hospital team in case management and discharge planning functions for patients in an inpatient setting. The role facilitates the continuum of care and the throughput process of transitioning patients to the appropriate level of care while ensuring the most cost‑effective and safe environment.
Qualifications- Experience:
Minimum one year of case management experience, preferably in the mental health field. Thorough understanding of the inpatient workflow process and step‑down process is preferred. - Education:
High school diploma or equivalent required;
Bachelor’s degree in a related field preferred. - Licensure/Certification:
Case Management Certification preferred. - Working Knowledge:
Good clinical skills to assess dynamics accurately and develop effective treatment strategies within the overall treatment plan, good analytical ability, and leadership abilities to support clinicians, nurses, and care workers. - Technical/Motor
Skills:
Computer software and keyboard proficiency; ability to operate copier, scanner, fax machines, phone systems, etc. Must be able to bend, lift, and reach office supplies and files. - Physical Requirements:
Physical agility to manage patients; ability to push, pull and lift up to 40 pounds; intermittent sitting, standing, walking, bending, and stooping.
