Healthcare Marketing – Community Liaison

• Develop and maintain strong relationships with hospital discharge planners, case managers, social workers, physicians, rehabilitation centers, skilled nursing facilities, assisted living communities, senior centers, Veterans organizations, and community partners.

• Schedule and conduct regular face-to-face meetings with referral sources throughout Cameron and Hidalgo Counties.

• Educate healthcare professionals, families, and community organizations about Com For Care 's services and how we help clients safely remain at home.

• Promote Com For Care  Home Care as the preferred provider for private-pay home care, Veteran home care services, and long-term care insurance clients.

• Identify new referral opportunities and develop strategic partnerships throughout the Rio Grande Valley.

• Represent Com For Care  Home Care at networking events, health fairs, educational presentations, and community outreach activities.

• Maintain consistent follow-up with referral partners to build trust and long-term relationships.

• Collaborate with office staff and caregivers to ensure a seamless experience for clients and referral sources.

• Track outreach activities and referral development efforts.

• Meet established outreach goals while helping grow referrals and community awareness.

• Excellent communication and relationship-building skills.
• Professional, confident, and comfortable meeting with healthcare professionals.

• Reliable transportation with the ability to travel throughout Cameron and Hidalgo Counties.
