POSITION SUMMARY:

The Physical Therapist Team Lead shall be responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualifications, professional practices and ethical standards. The Physical Therapist Team Lead shall also demonstrate accountability for and contribute to program development, quality improvement, and problem solving and productivity enhancement in a flexible interdisciplinary fashion.

The Physical Therapist Team Lead coordinates, delegates and supervises responsibilities assigned to Physical Therapists and supportive staff (PTA, therapy technicians, front office staff). Provides input in formulating budget, assists in evaluating department performance versus budget controls and takes appropriate action to remain in budget guidelines. Ensures that all Physical Therapy documentation and coding is accurate and entered on a timely basis.

Participates in all infection control, department equipment training, regulatory compliance training, organizational safety and fire programs.

• Graduate of accredited school of Physical Therapy required
• Current and active Physical Therapist License with the State of Texas required
• BLS CPR Certification is required
• Outpatient Orthopedic Physical Therapy experience preferred
• Ability to read, write and speak English
• Ability to communicate clearly and concisely with all levels of rehabilitation, administration and physicians

• Three (3) to five (5) years’ experience in Orthopedic Physical Therapy is strongly preferred.

• Promotes the facility mission, vision and values by effectively communicating them to others. Considers mission, vision and values in developing services, standards and practice
• Responsible for the supervision of Physical Therapy staff, including Physical Therapist Assistants and Aides.
• Conduct initial assessment. Choose appropriate assessment tools, adaptive equipment and material used in the assessment based on the age and developmental level of the patient.
• Patient assessment is completed and in the medical record as outlined in policy & procedure. Data is recorded in the appropriate areas.
• Develop individualized treatment plans. Short and long term goals are written objectively and are measurable and updated weekly.
• Time frame for goal achievement is documented.
• Interventions are selected/documented based on assessment results and are appropriate for age, cognitive abilities and/or development level of the patient.
• Interventions are selected/documented and applied to support the transdisciplinary rehabilitation plan as evidenced by goal achievement.
• Consults with patient’s physician regarding referrals for other services as needed.
• Patient response to the treatment plan is evaluated as evidenced by patient achievement of goals and/or goal revision, the treatment plan is updated as needed and/or on a…
