Position: Nurse III – PNMT Nurse (Physical and Nutritional Management Team Nurse)

Shift: Day (no telework) – up to 5% travel

The State Supported Living Center in Harlingen serves individuals with intellectual and developmental disabilities who are medically fragile or have behavioral problems. As a PNMT Nurse you evaluate assessment information, develop and implement physical and nutritional management plans, and collaborate with the interdisciplinary team to optimize the Individual Support Plan.

• Attend work regularly and may be required to work rotating or extended shift schedules.
• Participate in daily state‑center meetings to monitor individuals’ health status and identify when to refer to PNMT.
• Conduct specialized nursing assessments and develop PNMT recommendations with measurable goals and action plans.
• Develop and document individual health care plans for active PNMT caseloads.
• Perform post‑hospitalization assessments/reviews and report findings to PNMT.
• Collaborate with the IDT to integrate PNMT recommendations into the Individual Support Plan.
• Monitor effectiveness of PNMT interventions and make recommendations for improvement.
• Assist with treatment, interventions, and monitoring of individuals on active caseloads, including positioning during feeding and swallowing assessments.
• Collect, analyze, and report PNMT data for performance improvement and policy development.
• Serve in on‑call rotations and respond to emergency situations as required.
• Support disaster response or continuity of operations as needed.

• Extensive knowledge of nursing techniques, standards, procedures, and medical diagnoses.
• Knowledge of nursing care standards, the Settlement Agreement, and evidence‑based practices for individuals with developmental disabilities.
• Knowledge of ICF‑IID rules and emergency procedures including CPR.
• Knowledge of wound and ostomy care and root‑cause analysis.
• Computer and electronic health record skills, including data graph creation.
• Strong written and verbal communication for policy development and staff training.
• Ability to maintain CPR certification and use restraint safely.
• Ability to work independently, prioritize, and motivate staff.
• Physical strength and flexibility to support CPR and patient care.

Licensed registered nurse in Texas (or a state recognized by the Nurse Licensure Compact).

• Minimum one year of registered nurse practice experience.
• Preferred two years in a health agency, hospital, or health‑care facility.
• Preferred Bachelor’s degree.
• Experience with assessment and documentation of health status.
• Preferred experience with individuals having complex physical and nutritional needs.
• Preferred experience with individuals with intellectual or developmental disabilities.
