What you would be doing:

As the Central Intake Coordinator, you will oversee the referral management process to help coordinate and ensure census and case mix goals are achieved. You would work with the hospital liaison and marketing team, discharge planners and social workers at our referring hospitals, and the internal admissions team and leadership in our facilities to coordinate, schedule, and facilitate admissions. You would be working under the supervision of the Director of Post-Acute Transitions.

You would work typically work during business hours Monday through Friday with flexibility to work nontraditional hours. Some weekend on call coverage may be required.

• Preferably has existing one year experience in a customer service role
• Preferably has experience working in a Skilled Nursing Facility
