Abound Health is seeking a Contract Licensed Practical Nurse (LPN) to join our clinical services team in licensed residential homes and community settings.
This hourly, non-exempt contract role reports to the Director of Nursing, Program Director, or designated Clinical Supervisor. The LPN provides direct nursing care and clinical oversight to individuals receiving services under the Pennsylvania ODP waiver, ensuring health, safety, and regulatory compliance in accordance with 55 Pa. Code Chapter 6400.

Key Responsibilities
  • Provide direct nursing care in accordance with ISP, ICP, physician orders, and agency policy.
  • Monitor health status, conduct nursing assessments, and report changes to the RN and program leadership.
  • Complete and review HRSTs and support implementation of health risk interventions.
  • Review, update, and implement ICPs aligned with ISP outcomes and medical needs.
  • Support medication administration compliance through EMAR review and follow-up.
  • Document nursing care and report incidents in compliance with Chapter 6400.
  • Provide clinical guidance and health‑related training to direct support staff.
Qualifications
  • Current Licensed Practical Nurse (LPN) license in good standing in the state of practice.
  • Associate Degree in Nursing or higher.
  • Experience working with individuals with intellectual and/or developmental disabilities preferred.
  • Familiarity with ODP services, Chapter 6400 regulations, HRST, ICPs, ISPs, and medication administration.
  • Strong clinical assessment, documentation, and communication skills.
  • Ability to work independently and collaboratively within an interdisciplinary team.
Education

Associate Degree in Nursing or higher required.

This position operates primarily in licensed residential homes and may include community-based settings such as medical appointments and other public locations.

Compensation is determined based on experience, credentials, and job‑related skills. Contract roles are not eligible for benefits unless otherwise specified.
