Job SummaryGeisinger is proud to announce a salary increase for RN Hospital Case Managers – a perfect time to join us. This in‑person RN position is based at Geisinger Medical Center (GMC) and provides on‑site support to inpatient care teams.
Job DutiesAs a Hospital Case Manager, you will act with purpose and urgency to deliver timely, compassionate care, enhancing the patient experience. Your work supports innovative, collaborative, and responsive nursing practice.
Typical WorkweekMonday through Friday, 8:00 AM – 4:30 PM. Occasional shared weekend coverage, approximately every fourth weekend.
Qualifications- Minimum 2 years of RN experience.
- Prior RN experience in a hospital setting.
- Case Management experience is preferred but not required.
- Must be available to work on‑site at Geisinger Medical Center (GMC).
- Integrate evidence‑based clinical guidelines to develop patient‑centric treatment plans.
- Develop systems of care to monitor progress and promote early intervention.
- Assist with design, implementation, and evaluation of an advanced patient‑centered care model.
- Assess the healthcare, educational, and psychosocial needs of patients or members.
- Design an individualized plan of care and foster a team approach with the entire care team.
- Continuously evaluate laboratory results, diagnostic tests, utilization patterns, and other metrics.
- Apply benefits and utilization management, serving as a resource to the patient, member, and healthcare team.
- Maintain required documentation for all case‑management activities.
- Collect required data and use it to adjust treatment plans as needed.
- Graduate from Specialty Training Program‑Nursing (Required).
- Bachelor's Degree‑Nursing (Preferred).
- Minimum of 3 years of related work experience (Required).
- Licensed Registered Nurse (Pennsylvania) – of Pennsylvania.
