** Job Details
** With a commitment to patient- and family-centered care, the LPN uses the nursing process in ways that values the uniqueness of  each individual and addresses the physical, psychological, emotional, and social needs of the diversity of patients served by  Jefferson Health. The LPN is prepared to function as a member of the health-care team by exercising sound nursing judgment  based on preparation, knowledge, experience in nursing and competency.

The LPN participates in the planning, implementation  and evaluation of nursing care using focused assessment. Under the direction and supervision of the licensed clinical team  member, LPNs carry out selected nursing acts for patients that effectively address the individual needs and priorities of the  patient and are consistent with their cultural beliefs and values.

* Integrates Evidence-Based Practice, clinical expertise, patients, their family/designated care partner(s) preferences and cultural beliefs and values for delivery of optimal healthcare.
* Partners with patients, their family/designated care partner(s)) in identifying learning needs and priorities and provides education in ways that are useful, understandable, and affirming.
* Recognizes the patients, their family/designated care partner(s) in providing safe, compassionate and coordinated care based on respect for patient's personal preferences, priorities, needs, and cultural beliefs and values.
* Functions effectively within nursing and inter-professional teams, fostering open communication, mutual respect, and shared decision-making to achieve quality patient care.
* Creates an environment of belonging and inclusion, demonstrating skills in problem solving and conflict resolution.

* Uses data to monitor the outcomes of care processes and uses improvement methods to design and test changes to continuously improve the quality and safety of patient care.
* An LPN shall communicate with a registered nurse, physician, and the patient’s health care team members to seek guidance with patient care.
* An LPN may collect data as allowed by the licensed practical nurse scope of practice as defined by the
* Pennsylvania/New Jersey Nurse Practice Act
* Work closely with the registered nurse to assure the implementation of safe, quality care of the emergency department
* patients.

QUALIFICATIONS  Education - Required  
• Specialized Diploma Graduate of an accredited School of Practical Nursing and passed the Pennsylvania Board  Association for Licensed Practical Nurse.  Knowledge,
