RN per diem hours at Einstein campus Tier
2. Minimum commitment 24 hours. Weekend requirement 24 hours (included in minimum). Holiday requirement 1 winter & 1 summer.
With a commitment to patient- and family-centered care, the professional nurse uses the nursing process to value the uniqueness of each individual and address the physical, psychological, emotional, and social needs of the diversity of patients served by Jefferson Health Ambulatory Clinics. The nurse is an integral member of the ambulatory care clinical team, collaborating with providers and support staff to deliver clinical nursing interventions.
Responsibilities- Integrate evidence-based practice with clinical expertise, patient, family preferences, and cultural beliefs for optimal healthcare delivery.
- Collaborate and communicate with clinic care providers regarding patient care and management, supporting efficient and timely communication between primary care providers and specialists.
- Support implementation of clinical care pathways for common care conditions and actively participate in quality improvement opportunities.
- Utilize evidence-based ambulatory care nursing interventions during clinical visits, telephone encounters, and electronic communication.
- Provide patient and family education for preventive care, chronic disease management, medication management, and management of common illnesses treated in the primary care setting.
- Document all patient interactions and clinical care in the electronic medical record (EMR) in compliance with organizational standards and regulations.
- Perform care coordination tasks including abnormal results, triage, RN verification of vaccinations and injections, and management of in-baskets (prior authorizations and forms).
- Oversee clinical care essential to assessment, promotion, maintenance, and restoration of patients’ health and wellness.
- Minimize risk of harm to patients and families by following established nursing practices and protocols.
- Use information technology to communicate, manage knowledge, and mitigate errors.
- Use data to monitor care outcomes and support ethical decision making; apply improvement methods to design and test changes to continuously improve quality and safety of patient care.
- Function effectively within ambulatory care inter‑professional teams, fostering open communication, mutual respect, and shared decision making for quality patient care.
- Recognize patients and families as partners, providing safe, compassionate and coordinated care based on their preferences, priorities, needs, and cultural values.
- Create an environment of belonging and inclusion, demonstrating problem-solving and conflict-resolution skills.
- Commit to learning and pursue opportunities for professional growth and development.
- Actively participate in team huddles and assemblies to provide announcements and clinical operations updates.
- Demonstrate willingness to give and receive feedback and integrate appropriate changes into practice.
- Graduate of an accredited school of nursing.
- Knowledge of patient- and family-centered practice, including respect, collaboration, and care planning.
- Knowledge of cultural awareness and ability to connect with diverse backgrounds.
- Effective communication skills: ability to transmit, receive, and interpret ideas, information, and needs using appropriate behaviors.
- Professionalism: honesty, confidentiality, integrity, attention to detail, time management, and adherence to HIPAA and ANA conduct.
- Willingness to precept nursing students.
- Patient assessment and triage skills, including nursing and medical terminology.
- Patient and family education knowledge of child and adult techniques and health literacy barriers.
- Medication administration and pharmacology knowledge for primary care practice, immunizations, and preventive care.
- Assessment and diagnosis knowledge: use of tools and techniques for patient assessment, nursing diagnosis, and clinical judgement.
- Health information documentation skills: record medical histories, vital signs, medications, diagnoses, treatment plans, procedures, tests, and results; proficiency in EMR use.
