Job Description
Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. As a Maternal Child Health Nurse, you'll have the opportunity to make a lasting impact on mothers, newborns, and families by providing compassionate, one-on-one care in the comfort of their homes.
What You’ll Do as a Mom/Baby Registered Nurse- Provide skilled nursing care to expectant mothers, postpartum mothers, newborns, and high‑risk infants in the home setting
- Perform comprehensive assessments and develop individualized care plans
- Educate families on prenatal care, newborn care, feeding, safety, and overall wellness
- Monitor maternal and infant health and communicate changes to the care team as needed
- Collaborate with physicians and interdisciplinary team members to ensure quality patient outcomes
- Document patient care accurately and timely
- Support and empower families during pregnancy and the postpartum period
