• Health Monitoring:
  Observe and report changes in physical condition and behavior.
• Medication Administration:
  Administer prescribed medications and treatments according to policy.
• Clinical Support:
  Assist with diagnostic procedures, provider rounds, and collection of specimens.
• Care Coordination:
  Communicate with team members to support continuity of care.
• Program Implementation:
  Follow individual health plans and support personal care routines.
• Safety Practices:
  Participate in emergency responses and maintain compliance with required training.

• Full-time employment.
• 2nd shift (2:00 PM to 10:30 PM) with rotating break days and every other weekend off.
• Travel and overtime as needed.

• Active authorization to practice as a Licensed Practical Nurse in Pennsylvania.
• Employees with an active temporary practice permit must obtain licensure as an LPN within one year.
• PA residency requirement is currently waived.
• Eligible if within 60 days of completing an approved nursing program; must provide proof of current PA temporary practice permit or LPN license at appointment.
• Must be able to perform essential job functions.

• Conditional offer requires satisfactory completion of a medical examination and drug screening test.
• Position falls under the provisions of the Older Adult Protective Services Act, requiring submission and approval of satisfactory criminal history reports.
